Nurses' Notes — Day 1, Outpatient Mental Health Clinic
1000: Client is a 29-year-old female who presents today with her roommate for a follow-up appointment. Client has a history of bipolar I disorder, previously stable on lithium. Roommate reports the client has only slept 2-3 hours a night for the past week but "has more energy than ever." Client is dressed in brightly mismatched clothing and heavy makeup, speaking rapidly and jumping between topics. Client states, "I've never felt better in my life! I quit my job on Monday because I'm starting my own company — I'm going to be a millionaire within the year." Client reports she recently opened 4 new credit cards and has already spent over $8,000 "on things the business needs." Roommate expresses concern that the client has been "messaging strangers online and inviting them to come stay at our apartment." Client admits she stopped taking her lithium "about 3 weeks ago" because she "felt fine and didn't need it anymore." Client denies current alcohol or illicit substance use.
